Q - What is the Community Reintegration Program?
A - The Community Reintegration Program is designed to assist people with disabilities who meet particular eligibility and criteria with moving out of nursing homes and back to community living. The people are often financially "trapped" in the home because the medical need has ended but their income in benefits goes for their nursing home costs. The nursing home social worker often refers people with disabilities in this situation to PACE.
Q - Who is eligible for the program?
A - Any consumer who has a disability residing in an Intermediate Care Facility (ICF) or Skilled Nursing Facility (SNF) operating under the Nursing Home Licensing Act that is not funded by the offices of Developmental Disability or Mental Health.
Q - What are the criteria for the Community Reintegration Program?
A - Any consumer with a disability who is living in an ICF or SNF under the age of 60 is eligible for the program. The consumer must reside in an ICF or SNF for at least 90 days and be Medicaid eligible or show proof of Medicaid approval/denial. A score of 29 or above is required on the Determination of Need Assessment to qualify for the program.
Q - Does the program only serve people with disabilities under the age of 60?
A - No. A person with a diagnosis of HIV/AIDS or Traumatic Brian injury is eligible for the program regardless of age.
Q - What services are provided by the Community Reintegration Program?
A - Services provided by the Community Reintegration Program include:
&bull Assistance with locating accessible/affordable housing
&bull Assistance with transportation/moving
&bull General Support Services
&bull Help in obtaining personal assistant services in the home *Assistance with start-up groceries
&bull Assistance with obtaining furniture and household items
&bull Skills Training
Q - How many times can I utilize the Community Reintegration Program?
A - The Community Reintegration Program allows for a one-time move from an IFC or SNF.
Q - How long are services good for?
A - A yearly review is completed on each person who has moved back in to the community to determine if services are still necessary.
