What are the Eligibility Requirements to receive a free amplified phone?
You must be a legal resident of the State of Illinois
You must have working land line telephone service at the address you show on your application form.
You must be certified as Deaf, Hard-of-Hearing, Speech-Disabled or Deaf-Blind by:
Licensed Physician,
Speech-Language Pathologist,
Audiologist,
or designated DHS/DRS Counselor.
* There are no age or income restrictions to qualify for the programs

Those qualified will receive a voucher which entitles them to a free amplified phone.
There are MANY different types and degrees of hearing loss. Because a phone works for one person does not mean it will work for another. There are various differences in tone and clarity that must be matched to the users needs. The only proven way to do this is to try the phones on a "real" call. PACE and other selection centers throughout the state are set up to help you do this.
You must visit a Selection Center like PACE to make the best choice for YOU.
